About the Department St. Vincent’s Mental Health (SVMH) includes an adult area mental health service for the inner city areas of Yarra and Boroondara. About the Role A rare and exciting opportunity has arisen for an experienced Mental Health Clinician who has a keen interest in joining the Clarendon Homeless Outreach Psychiatric Service (CHOPS) Team based at Clarendon Community Mental Health Clinic. You are looking for a community based role in an innovative environment, working with clients who may be homeless or at risk of homelessness who suffer with a serious mental illness and associated high level of disability. You are seeking a service that is committed to utilising a recovery-orientated model of care (Strengths Model). You will work within a Multidisciplinary Team where you will be exposed to a broad range of clinical work with some of the most disadvantaged clients. In addition to the clinical work a significant component of the role involves the development and maintenance of sustainable working partnerships with a range of other service providers both internal and external to the St Vincent’s’ Health organisation. Classification Range OT/SW Grade 2 YB20 YB23 or YC42 YC45 ($81,328 - $93,490 per annum) RPN Grade 3 NP81 NP74 ($91,000 - $101,056 per annum) Duties Work within the Strengths Model of recovery orientated practice. Provide effective case management to identified consumers including secondary caseload support for other case managers as required. Build a therapeutic alliance with the consumer and family/carers and actively engage them in the planning, delivery and evaluation of their care. Undertake the assessment, provision of treatment and delivery of targeted interventions to consumers with complex mental health related needs and their family/carers in line with the homeless team model of care and SVMH policies and strategic direction. Utilise discipline-specific skills as appropriate and as required by consumers, their family/carers and the Service. Work collaboratively within the multi-disciplinary team and contribute effectively and appropriately to clinical review processes and team activities. Qualifications, Skills and Experience A minimum of at 2 years post qualification clinical experience within Mental Health. Registration with Australian Health Practitioner Regulation Agency (all disciplines except Social Work).
